Who We Are
Continuum Care Center (CCC) is a grassroots Non-profit organization led by members of the recovery community that provides resources to promote long-term recovery and improve the quality of life while building recovery capital. At CCC, we work with other community service providers and organizations to help the consumer link or navigate there barriers to recovery. We provide culturally specific peer support services and resources to the most vulnerable population. We offer non clinical services to support those in treatment centers, or other settings, ranging from treatment programs, criminal justice agencies, to shelters and other service organizations.
We are committed to providing a welcoming, safe, and supportive environment for those seeking recovery and during the initiation, ongoing, and post-acute stages of recovery. We recognize that each individual we serve comes from diverse socio-economic backgrounds, ethnic groups, gender identities, and religious beliefs. We are peer-driven with a majority of our staff and volunteers are from diverse communities and are in long-term recovery from substance use disorders. We are not professional counselors or therapists. We are people who have found a pathway to recovery that is meaningful, and we seek to share our hope and experience to support others who wish to find the same.
We strive to help people with barriers to live self-determined lives in their communities by providing recovery support, advocacy, and resources or referrals to other services that may be needed.
Executive Director
Eric Bacon. LADC is the CEO and Founder of Continuum Care Center. With a number of years serving the homeless population and those with SUDs in the community. He is in long-term recovery with over 25 years.
Samantha Walker, MS
Program Director
Samantha is the Program Manager at Continuum Care Center and has a MS in Human Behavior and has a number of years experience working with individuals experiencing SUDs and co-occurring mental health issues and is in long-term recovery.
Clifford Harris
Peer Recovery Coach
Cliff has 16 years in Recovery and went through Minnesota Recovery Connection's Peer Recovery Coach Training. A new addition to our team, he shows great skill in helping others, care, concern, and is now working on his certification.
Eric Bacon, LACD
Continuum Care Center is a Recovery Community Organization
We Are one of several Recovery Community Organizations in Minnesota. An a leader in the Recovery Outreach and Support in Community. We are an ally to other Recovery based organizations such as Minnesota Recovery Connection, Recovery is Happening, Recovery Community Network, and Twin cities Recovery Project. They also provide trainings for Recovery Coaching, and Volunteer Opportunities as well as a number of additional resources.
